South Bucks Primary Care Network
South Bucks PCN
A Primary Care Network (PCN) consists of groups of general practices working together with a range of local providers, including across primary care, community services, social care and the voluntary sector, to offer more personalised, coordinated health and social care to their local populations.
There are 12 PCNs across Bucks CCG.
PCNs are led by clinical directors who may be a GP, general practice nurse, clinical pharmacist or other clinical profession working in general practice.
Ivers Medical Centre is part of the South Bucks PCN made up of the following five practices:
- Iver Medical Centre
- Threeways Surgery
- Burnham Health Centre
- Southmead Surgery
- Denham Medical Centre
The South Bucks PCN Clinical Director is Dr Daniel Djemal.
We share a Social Prescriber, a Clinical Pharmacist, Pharmacy technicians and a first contact Physiotherpaist as part of our wider team.
